What is your new patient procedure? *CURRENTLY NOT ACCEPTING NEW PATIENTS*
We are so excited to meet you! All new patients will experience an assessment as their first appointment.
Do I need a doctor’s referral?
In order to receive treatment at our clinic, you do not require a doctors referral. However keep in mind that some extended health benefits plans or claim coverage does require a prescription from your doctor. Please refer to your individual plan details for this information.
What types of health insurance do you accept?
At our clinic, we can direct bill for most extended benefits companies. Please refer to your individual plan to predetermine your approval.
All new clients are required to sign a direct billing agreement. We will do our best to submit and coordinate your billing for direct payment through your individual company, however if there is a problem with a claim, you are responsible for settling your balance with us at Equus Physio.
